Thank you for your interest in receiving assistance from the Creek's Helping Hands Serve Team. We assist widows, single mothers, the elderly, and others who need help with various tasks. Our ability to assist with a project is determined by the scope of work that is needed, the number of active requests and the number of volunteers available to help.

I understand that while the Helping Hands Serve Team is at my residence, myself, or an adult representative above the age of 18yrs old must remain at the residence for the entirety of the work performed. *

I understand that my request will be reviewed by a Team Leader who will reach out to help determine the amount of work and materials that will be needed to complete the request. A site visit may be required to make the final determination. *

I understand that the labor will be provided by the Helping Hands Team and that if there is a cost associated with the repair for supplies and materials to complete the requested project, then I will be responsible for covering those costs. *

I UNDERSTAND THAT THE HELPING HANDS SERVE TEAM IS UNABLE TO FULFILL EMERGENCY REQUESTS. *

I hereby release The Creek Church and any volunteer from all claims, liabilities, actions, damages, and future expenses associated with this project. *

I understand that this Release shall be a FULL AND COMPLETE RELEASE of The Creek Church, of any liability for past, present, and future actions. I further declare and represent that the terms of this Release are contractual and not a mere recital.

The Helping Hands Serve Team, WILL NOT accept any monetary transaction for work performed.
